Vacation Leave Form

If you know your child will be missing school for a trip, please fill out a vacation form at least a week in advance.  This is important for recording purposes and so that we can get work ready for the upcoming trip. You may pick up a form from the front office or download the pdf below and email it to your classroom teacher or drop it off at the front office.
